I am seeing new patients ages 3 and up in Menlo Park and virtually. I don’t accept any insurance other than Lyra Health through participating employers.
Please request a time for a brief complimentary phone consultation and complete the pre-screening questionnaire below. This allows me to quickly understand whether my practice is a good fit for you or your child. If you are scheduling for your child, they do not need to be present for the phone call.
All information you submit is encrypted and transmitted through HIPAA-compliant systems. Appointment requests are reviewed individually and are not confirmed until you receive a response from me.
If no time slots are available, then I am not currently accepting new patients.
